There is no simple answer here. The point is to consider such fundamental questions as

  1. What is the purpose of health care (to make money or provide for the health of the population)?
  2. Who should make such decisions?
  3. Is health care a right or a privilege?
  4. Should criteria for health-care decisions be based on the ability to pay (either higher insurance premiums or additional patient costs)?

Health care decisions are seldom made in a vacuum, but it is these very real sorts of economic, social and humanitarian considerations that health care professionals must increasingly face today.
